Step 1: Self-Survey

Identify current habits or choices that you may want to change.

Step 2:
Action Plan
Create an individualized plan for making healthier choices. Identify obstacles and develop solutions to overcome those obstacles.

Step 3: Tracking

Using the specially designed 20-Day Health Challenge tracking poster, strive to create healthy habits for 20 days.

Step 4:
Reflection
Identify why you were successful and plan for the long term.
